I can't get past the fact that they named him Tosh. As much as I love Tosh.0, that is the only thing I can think of when I look at him.

Omg, its Karev. I kinda hate him sometimes, but he can be charming in grey's anatomy. But no, not gonna buy.

Looks to realistic ,not c/p tho. I see some differences ,like hairs on his chin, he has no hair on chin at all ,but he might be just filtrated. You can simply get this looking using filter Dust and Scratches ,Radius 5px and Threshold 0 levels ,then making opacity lower (60-70%) ,then merging layers and resizing picture. Then they probably apply Sharpen more filter to make it more sharp. After then they maybe use posterize ,level 4 ,and then ,when layer shows up ,they make brush opacity lower ,and then ,depends how much you want to make it cartoonish at that place ,press longer or shorter. Then you may apply despeckle filter on duplicate of that layer ,then 30% opacity ,and screen bleeding. A little lower brightness and contrast to take off light look. Smudge a bit on some places to make smudgy look ,like you were smudging to make him look shaded. Then use burn tool and cover some parts that still look realistic. Then change bleeding option to luminosity on layer before despeckle and put it over one you are working on currently. Merge down. Then use smart blur filter ,radius 76.4 ,Threshold 14.2 (may around ,don't need to be exact). Then you'll notice that eyes look most realistic. Use crystalize filter on duplicate of that layer. Erase everything that are not eyes on that layer. Then change bleeding to luminosity ,then change opacity to 40%. Then unsharp mask ,amount 20 ,radius 250.0 px Threshold 0 levels. Make duplicate of layer ,use filter again. Change opacity to 30%. Then smudge some parts a bit more and you're done. Here you go ,see difference:

They might be removing pixelated edges by bluring and adjusting him but that should be it. I guess they have shortcut as my Photoshop is older version. Aren't we amazing artists?

:/ I woulda expected a tattoo artist to have more visible tattoos. I suppose he could have several hidden away on *ahem* let's say less visible places, but anyone who is selling a craft I would think would want said craft visible and on display. The guy should be inked up on his shoulders, maybe one curving around his neck...come on I know Disney stays PG and PC but it's not like he has to be a 'bad boy' tattoo artist. Tasteful tats can look good. He just looks like a flop of a tattoo artist.
